He outlines the current use of AI in mental health from low-risk administrative support (transcription, summaries, risk-language flags) to adjunctive tools (CBT exercises, mindfulness, psychoeducation, mood tracking) and, more controversially, AI positioned as a primary provider via chatbots and companion apps.
He reviews mixed research: digital CBT can help mild to moderate anxiety/depression (e.g., Woebot) but lacks credible evidence and is inappropriate as a primary intervention for complex conditions like trauma, severe depression, suicidality, psychosis, complicated grief, and relational trauma.
He highlights ethical concerns: inadequate informed consent, lack of scope-of-practice safeguards, crisis-response failures, and absence of a true therapeutic alliance. He argues AI should amplify clinicians, not replace them, and previews part three on reclaiming a human edge in an AI-abundant world.

The episode explains the “fawn response,” a trauma/stress survival strategy first articulated by Pete Walker in complex PTSD work, in which perceived relational threat triggers appeasement and fear-based compliance when fight or flight feel unsafe. Dr. Vergolias describes how fawning can be rewarded in leadership and corporate contexts yet leads to burnout, resentment, and loss of identity. He summarizes the neuroscience through Stephen Porges’s polyvagal ventral vagal social engagement system being co-opted by threat, with costs such as elevated cortisol and dissociation.
He lists behavioral markers (difficulty saying no, over-apologizing, anxiety about disappointing others, conflict-smoothing, resentment) and offers four practices: pause before agreeing, reconnect to body signals, separate values from fears, and tolerate others’ disappointment through gradual exposure.
